- Visits one of our locations in Oakbrook or Arlington Heights
- Or meet with your provider from home using secure video
Either way, the care is the same: private, respectful, and centered on what works for you.

You should always contact your insurance provider before booking to verify we are in-network and if authorization is needed.

- All groups besides Amita and Loyola will need an authorization from your primary care provider
- Blue Cross/Shield Community Health

- Cigna, higher copayment will apply
We are in other networks, please check with your insurance company using the individual provider’s name
